This intake form typically takes 15–25 minutes to complete. Before you begin, please gather your employee information, tax account numbers, and recent payroll records (if applicable).
Be sure to hit “Submit” once you’ve finished to ensure we receive your information. Closing the browser before submitting will result in lost data, and you’ll need to re-enter your information.
Please remember to upload all required documents securely before submitting. Once completed, schedule your onboarding call so we can review your setup, answer any questions, and finalize your payroll processing.
Business Information
Please complete all fields with your company’s legal information. This ensures your payroll and tax records match what’s registered with the IRS and your state.
If your business is newly formed and some information isn’t available yet, we’ll guide you through completing any missing steps during onboarding.
Contact Information
List the person(s) responsible for handling payroll, taxes, or HR-related matters. This ensures we can reach the right contact for approvals, updates, and time-sensitive notices.
If you have a secondary contact, include them as backup in case the primary contact is unavailable.
Payroll Setup
Provide details on your pay schedule, preferred start date, and pay delivery method. Accurate setup helps us ensure smooth payroll processing and compliance with filing deadlines.

Employee Information
Include the total number of employees and note whether you have W-2 employees, 1099 contractors, or both.

Please provide your federal and state tax account information below. These details are required for accurate payroll setup and tax filing.
If you do not currently have one or more of these accounts, no worries. New Lyfe Financial Group will assist you with the application process as part of your payroll setup.
